CEOWorks Business Health Assessment Giveaway — Official Rules

1. Promotion Name

CEOWorks Business Health Assessment Giveaway (the “Giveaway”).

2. Sponsor

The Giveaway is sponsored by CEOWORKS, LLC, a Florida limited liability company doing business as CEOWorks, located at 7901 4th St N, Ste 300, St. Petersburg, FL 33702 (“Sponsor,” “we,” “us”).

3. Eligibility

Open only to legal residents of the fifty (50) United States and the District of Columbia who are at least eighteen (18) years old at the time of entry. Employees of Sponsor and their immediate family members (spouse, parent, child, sibling, and each of their spouses) or household members are not eligible. Void where prohibited by law.

4. Promotion Period

The Giveaway begins on October 2, 2026 at 12:00 PM Eastern Time and ends on October 11, 2026 at 11:59 PM Eastern Time (the “Promotion Period”). Entries submitted outside the Promotion Period will not be counted. This is a public promotion window surrounding the FedEx Summit; FedEx Summit attendance is not required to enter during this window or at any time.

5. No Purchase Necessary

No purchase, payment, donation, or Summit ticket is necessary to enter or win. A purchase or payment of any kind will not increase your chances of winning.

6. How to Enter

During the Promotion Period, visit ceoworks.io/business-health and: (1) complete the CEOWorks Business Health Assessment; and (2) at the contact step, affirmatively check the box to opt in to the Giveaway and confirm you have read and agree to these Official Rules. Completing the Assessment alone does not enter you in the Giveaway — both steps are required. Attendance at the FedEx Summit is not required to enter or win, and entry is open to the public online for the full Promotion Period regardless of Summit attendance.

7. Entry Limit

Limit one (1) entry per person, regardless of the number of Assessments completed, email addresses used, or devices used. Sponsor may disqualify duplicate or multiple entries from the same person.

8. Prizes and Approximate Retail Value

First Prize: Coleman Snap 'N Go 45-Quart Collapsible Hard Cooler — Approximate Retail Value $219.99.
Second Prize: Soundcore Motion X600 — Approximate Retail Value $159.99.
Third Prize: $50 Amazon Gift Card — Approximate Retail Value $50.00.
Total approximate retail value of all prizes: $429.98.

9. Odds of Winning

Odds of winning depend on the total number of eligible entries received during the Promotion Period. Completing the Assessment, attending the FedEx Summit, scheduling a call with CEOWorks, purchasing or engaging CEOWorks' services, or providing marketing consent do not improve your odds of winning.

10. Winner Selection

Three (3) winners will be selected in a random drawing from all eligible entries received during the Promotion Period, conducted by or on behalf of Sponsor within three (3) business days after the Promotion Period ends. Selections are final, subject to eligibility verification.

11. Winner Notification

Potential winners will be notified using the email address submitted with their Assessment entry. Sponsor is not responsible for lost, delayed, or misdirected notifications, including due to spam filtering.

12. Response Deadline

A potential winner must respond to the notification within five (5) business days of the date it is sent. Failure to respond within this period, or a return of the notification as undeliverable, will result in disqualification and selection of an alternate winner in that prize position.

13. Alternate Winners

If a potential winner is disqualified, does not respond in time, declines the prize, or is found ineligible, Sponsor may select an alternate winner for that prize position from the remaining eligible entries, using the same random selection method. This process may repeat until a prize position is filled or a reasonable number of alternates has been exhausted, at Sponsor's discretion.

14. Prize Delivery

Sponsor will ship prizes to confirmed winners at no cost to the winner, to a shipping address collected only after a winner is confirmed. Shipping addresses are not collected from entrants who do not win.

15. No Cash Substitution

Prizes are not transferable and no cash substitution is permitted, except that Sponsor reserves the right, in its sole discretion, to substitute a prize of equal or greater value if the advertised prize becomes unavailable for any reason.

16. Taxes

Winners are solely responsible for any applicable federal, state, or local taxes associated with acceptance and use of a prize.

17. Verification of Eligibility

Sponsor may require a potential winner to verify their identity, eligibility, and compliance with these Official Rules before awarding a prize. Sponsor reserves the right to disqualify any entrant or potential winner it reasonably believes has violated these Official Rules, tampered with the entry process, or engaged in fraudulent activity.

18. Release and Limitation of Liability

By entering, participants agree to release and hold harmless Sponsor and its owners, employees, and agents from any claims, losses, or damages arising from participation in the Giveaway or the acceptance, use, or misuse of a prize, to the fullest extent permitted by law. To the fullest extent permitted by law, Sponsor is not responsible for entries lost, delayed, or corrupted due to technical issues, including issues with the Assessment website or email delivery.

19. Privacy

Information collected in connection with the Giveaway is handled as described in Sponsor's Privacy Policy, including the giveaway-specific terms described there. By entering, participants agree to this data handling for purposes of administering the Giveaway, including winner notification and prize fulfillment.

20. Governing Law

These Official Rules are governed by the laws of the State of Florida, without regard to conflict-of-law principles, and any disputes will be resolved in the state or federal courts located in Miami-Dade County, Florida.

21. Void Where Prohibited

This Giveaway is void where prohibited or restricted by law. If participation from any jurisdiction is restricted, entries from that jurisdiction will not be eligible.

22. How to Obtain a Copy of These Rules

These Official Rules are available at ceoworks.io/giveaway-rules for the duration of the Promotion Period. A copy may also be requested by emailing support@ceoworks.io.

23. Winner Announcement

Winners' first name and city/state of residence may be shared on CEOWorks' website or social media only with the winner's separate, affirmative consent obtained at the time of notification — this is never a condition of receiving the prize.
